Our Mission Statement

We want to provide high levels of Complex Medical Care in a homely environment. Our goal is to focus on a small number of patients and to provide them with the best possible quality care. Our mission is to achieve an environment where our resident and their family see themselves not as patients but rather as members of our extended family. We strive to meet all their needs on a 24-hour, 7-day basis.

Our Vision Statement

Our vision is to redefine complex medical care by creating a nurturing, home-like environment where every resident feels valued and supported. We aim to foster meaningful connections between residents, families, and caregivers—built on trust, compassion, and respect. Through personalized attention and around-the-clock care, we strive to set the standard for excellence in congregate living.

Who We Are

We are medical professionals and noticed that a lot of our patients were not able to get a high level of care due to their medical needs. These patients were too sick for skilled nursing facilities and had multiple problems to be able to get good care. This led to multiple hospitalizations and did not offer the patients the optimum environment to heal. Having experience in assisted living facilities, we wanted to keep that homey feeling.

We are licensed by CDPH as a Type A/B/C facility. Our private rooms are featured with professional medical equipment, LED TVs, and a physical therapy room. Our facility is staffed with a 3:1 – 6:1 Licensed nursing and Certified Nursing Assistant ratios 24 hours a day. Our Nursing Director has a Master's in Nursing. All sub-acute and skilled nursing services are offered, led by our clinical nursing administration staff in collaboration with our specialty teams. These include Pharmacy, Dietitians, Physical Therapy, Occupational Therapy, Speech Therapy, Infusion Therapy, Specialty Wound Care, recreational therapy, and respiratory therapy. Our medical director is an infectious disease physician with extensive experience in IV antibiotics and complex wounds. We help take care of complex pulmonary patients who need chronic weaning from tracheostomy.
